The Rock County Public Health Department and Health equity Alliance of Rock County team up to complete a Community Health Assessment.
The purpose of the assessment is to understand the current health needs of those that work, live, and play in Rock County and to help identify where and how we can improve the health of our community.
Citizens are being asked to fill out a short survey about their personal and community health.
